ABSTRACT

OBJECTIVE: A prospective, randomized controlled trial was conducted to evaluate the efficacy of nerve-sparing radical hysterectomy (NSRH) in preserving bladder function and its oncologic safety in the treatment of cervical cancer. METHODS: From March 2003 to November 2005, 92 patients with cervical cancer stage IA2 to IIA were randomly assigned for surgical treatment with conventional radical hysterectomy (CRH) or NSRH, and 86 patients finally included in the analysis. Adequacy of nerve sparing, radicality, bladder function, and oncologic safety were assessed by quantifying the nerve fibers in the paracervix, measuring the extent of paracervix and harvested lymph nodes (LNs), urodynamic study (UDS) with International Prostate Symptom Score (IPSS), and 10-year disease-free survival (DFS), respectively. RESULTS: There were no differences in clinicopathologic characteristics between two groups. The median number of nerve fiber was 12 (range, 6 to 21) and 30 (range, 17 to 45) in the NSRH and CRH, respectively (p<0.001). The extent of resected paracervix and number of LNs were not different between the two groups. Volume of residual urine and bladder compliance were significantly deteriorated at 12 months after CRH. On the contrary, all parameters of UDS were recovered no later than 3 months after NSRH. Evaluation of the IPSS showed that the frequency of long-term urinary symptom was higher in CRH than in the NSRH group. The median duration before the postvoid residual urine volume became less than 50 mL was 11 days (range, 7 to 26 days) in NSRH group and was 18 days (range, 10 to 85 days) in CRH group (p<0.001). No significant difference was observed in the 10-year DFS between two groups. CONCLUSION: NSRH appears to be effective in preserving bladder function without sacrificing oncologic safety.

ABSTRACT

El carcinoma adenoescamoso de la ampolla de Váter es una neoplasia rara (incidencia de 1 por ciento de todas las neoplasias de la ampolla de Váter), en la que los componentes glandular y escamoso son histológicamente malignos. En el presente estudio reportamos el caso de una mujer de 61 años que presentó ictericia y dolor abdominal. Se le diagnosticó adenocarcinoma de la ampolla de Váter y fue sometida a una pancreaticoduodenectomía. Se realizó una resección, con ausencia microscópica de enfermedad residual (R0). El resultado final de patología fue un carcinoma adenoescamoso de la ampolla de Váter, T4N0Mx. Dos y medio meses después presentó recurrencia hepática múltiple, falleciendo al tercer mes de realizada la pancreaticoduodenectomía. Se hace una revisión actualizada de esta patología infrecuente y con pobre pronóstico.


Adenosquamous carcinoma of the ampulla of Vater is a rare neoplasia (1 per cent of incidence of all ampulla of Vater ´s neoplasias), in which both glandular and squamous elements are histologically malignant. In this study, we report the case of a 61 years old woman who presented jaundice and abdominal pain. The patient was diagnosed with adenocarcinoma of the ampulla of Vater and underwent pancreaticoduodenectomy. A R0 (no residual disease by microscopy) resection was performed. The final diagnosis was adenosquamous carcinoma of the ampulla of Vater, T4N0Mx. Two and one half months later she had multiple liver metastases and died 3 months after surgery. An updated review of this unusual disease and of its dismal prognosis is made.

ABSTRACT

Durante 12 años en el Instituto Nacional de Cancerología de México, se efectuaron 130 histerectomías radicales clase III de Rutledge o clásicas, con linfadenectomía pélvica bilateral (tipo Okabayashi-Meigs) para el tratamiento de pacientes con cáncer cervical 1-B-1. La edad media fue 46 años (rango 28.64). El tamaño tumoral fue menor de 2 cm. en 104 casos y obviamente en ninguno fue mayor de 4 cm. El tiempo quirúrgico medio fue 4:50 h. (rango 3-15). El sangrado transoperatorio (mortalidad operatoria 0.76 por ciento). En 8.4 por ciento se presentaron alteraciones de la función vesical y en 6.9 por ciento fístulas uro-vaginales. El diagnóstico histopatológico fue 113 carcinomas epidermoides, 10 adenocarcinomas, 6 adenoescamosos y una adenocarcinoma de desviación mínima (adenoma maligno). Presentaron metástasis ganglionar pélvica 10 por ciento de los casos. Recayeron 13 pacientes, 12 con carcinomas epidermoides y una con adenocarcinoma. En sólo tres de siete recurrencias fue útil la radioterapia de rescate. Otra paciente murió de causa no relacionada al cáncer. Han continuado vivas y bien 118 casos (media de seguimiento 63 meses)
